To find out what information we might have about you, we need some information from you. ie a description of the information you are looking for or a description of the circumstances in which you had contact with us.

If you have a reference number, dates and times or any other information you can give us which may help us in finding the information you're asking about, please include that here.
Your Request

Please upload scans, digital downloads or photos of two proof of identity documents.

You can use documents like a utility bill (which must be within three months old) or a valid photocard driving licence or passport. We're asking for these because we need to be sure you are who you say you are, due to the kind of information we may hold.

The two documents between them need to show your:
- name
- date of birth
- current address
- signature
- photo
Each file must be under 5MB in size and they must be one of the following file types: .jpg .jpeg
